Methodology. All figures are based on claims filed within the trailing 12-month window, rounded to the nearest 5 to prevent any inference about individuals. Days-to-payout counts only approved claims and is measured from the moment a claim was filed to the moment funds left our account. Denial numbers include any claim closed without payment, typically due to documentation incompleteness.
